What is a chronic illness?

Chronic illness is a medical term used to describe long-term health problems that usually aren’t curable. However, by diagnosing chronic illness early on, it’s possible to manage and prevent more serious complications in the future.

What is chronic disease management?

Chronic disease management is a branch of healthcare focused on diagnosing, treating, and managing chronic illness, such as diabetes, hypertension, and high cholesterol. By participating in chronic disease management, you can lower your risk of serious health problems, including heart disease, certain types of cancer, amputation, and premature death.

Not all primary care physicians in San Antonio offer a full suite of chronic disease management services. At Tailored MD, you can access treatment for all types of chronic diseases, allowing you to address all of your medical needs in one place. This saves you time and the hassle of having to see various specialists.

What is preventive healthcare?

Preventive healthcare is an important aspects of healthy living. Preventive refers to preemptive measures used to diagnose and treat health problems like high blood pressure and diabetes early on. Many chronic conditions don’t present symptoms at first. Holistic, integrative measures of care, such as regular exercise and clean eating, can restore balance to your body and, in turn, your overall health.

Healthcare isn’t something that just happens. It requires a conscious effort and willingness to change bad habits, such as smoking or living a sedentary lifestyle and replacing them with healthier alternatives.

Who can benefit from preventive healthcare?

Anyone who wants to lead a healthier, longer life can benefit from preventive healthcare. It provides you with the tools and insights you need to live your best life. What is a physical exam?

A physical exam is a routine evaluation of your general health. Also called an “annual exam” or a “wellness exam,” it allows Dr. Clements to note your current health and identify trends and patterns in your health over time. Furthermore, he can find signs of conditions affecting your health before they cause any symptoms. Physical exams also give you the chance to gain a better understanding of the way your body functions. You can ask Dr. Clements questions about your health, and he can recommend lifestyle changes that can help preserve your health over time.

Why might I need a physical exam?

Dr. Clements provides a variety of physical exams at his private practice. There are many reasons why you might need a physical exam, whether it be for yourself, the government, your coach, or your employer. Dr. Clements offers:

Annnual physicals

You should get a physical every year to monitor your body’s changes as you age. You don’t need to be sick or concerned about your health to benefit from an annual physical.

Pre-employment physicals

Many employers require that you get a physical exam as a condition of your employment. It may include a drug test, which you can take in the on-site lab. If you have symptoms, Dr. Clements can find a diagnosis and a treatment for it so you can work safely.

Sports physicals

Sports physicals, also called pre-participation physical exams, evaluate your health and your fitness. Dr. Clements looks for conditions that could make sports unsafe for you.

What is Obesity?

By definition, obesity is BMI over 30. BMI is calculated using height and weight. There are many online tools to use to calculate your BMI such as this one (Click Here). Overweight is defined as BMI between 25-30. A BMI is an important tool in measure an aspect of health but that is all it is. It does not define health and wellness and more than other health markers. Happiness, and comfort, and personal health and weight goals take precedence over any single scale such as BMI.

Treatment objectives

Some people may have a certain weight or BMI goal in mind that they would like to reach.  The doctors at Tailored MD are certainly here to help you with those goals and can utilize any of the tools and techniques listed below to help you along the way. Some people may have different goals, maybe they would like to attain a level of muscle mass and fat loss in order to play with their children or grandkids without getting out of breath. Maybe someone wants to focus less on food or has anxiety about food. Another individual may have weight or adiposity related diseases such as fatty liver disease, type two diabetes, hyperlipidemia and they would like to manage these conditions without adding or increasing medication. All of these are good reasons to get started with our San Antonio direct primary care services.

Treatment Options

Any successful weight or body fat loss program includes a dietary plan. As you may know, there are thousands of diets out there and no single diet is king. The Keto diet, Adkins diet, calorie counting, intermittent fasting, Paleo diet, are all effective for some people and not for others. The truth is, they all fundamentally work the same way. The key to long term dietary success is understanding how diet and metabolism effects weight. Sticking to a plan that fits your goals and lifestyles.  Exercise is also an important part of overall health and can effect weight, especially in keeping weight off, but exercise is a far number two when compared to diet.  In addition to diet and exercise, a Tailored MD doctor may recommend adjusting medications. For example, medications can be added to help lower appetite or effect glucose metabolism. Other medicines that you are taking may make weight loss harder and your Tailored MD physician will look over these to see if adjustments can be made.

Geriatric care is a branch of medicine that works to diagnose, treat, and prevent illness in adults age 65 and older. As you get older, your body experiences an array of changes. Although this is a normal part of the natural aging process, it also puts you at risk for certain health problems.

What services does geriatric care provide?

At Tailored MD, geriatric care involves longer appointment times and one-on-one consultations, allowing your provider to develop a custom-tailored care plan specific to your individual needs. In addition, geriatric patients don’t have to share a waiting room. This lowers the risk of disease transmission and provides for a more relaxing experience.
